      Frequently asked questions

      What is compounded semaglutide?
      Accordion toggle icon

Many people have turned to compounded medications in the face of supply shortages. Compounded medications are prepared by special pharmacies licensed to prepare medications at the request of healthcare providers.

Compounded medications are not generic medications.

Hello Alpha providers prescribe to a pharmacy that meets our high standards for providing high quality care. This pharmacy specializes in compounding semaglutide base and not semaglutide salts, which are not the same active ingredient in Ozempic and Wegovy. To ensure compliance with applicable law, we cannot send compounded prescriptions to other pharmacies.

Subject to availability and applicable law, Hello Alpha currently offers compounded semaglutide as multi-dose vials that require the use of a needle and syringe for weekly injections. Although many have used compounded semaglutide to safely perform self-injections, if you are not comfortable filling and using a needle and syringe to self-inject, ask your Hello Alpha provider for other alternatives.

      Is compounded semaglutide FDA-approved?
      Accordion toggle icon

The FDA allows compounding pharmacies to compound medications under certain circumstances, but it is important to note that compounded medications, including compounded semaglutide, are not FDA approved. The FDA does not verify the safety or effectiveness of compounded drugs.

What do I need to administer compounded semaglutide?
      Accordion toggle icon

Compounded semaglutide is not packaged as preloaded injector pens. The only way to administer compounded semaglutide is through an injection using a needle and syringe.

The medication will ship to you in a small glass vial. It is important that you self-administer each dose carefully and accurately. If you have hand or vision issues that may interfere with your ability to draw up the appropriate dose and administer the medication, please discuss these concerns with your provider. Alternative medications are available.

Each compounded semaglutide prescription delivery will contain a vial of compounded semaglutide, 10 injection syringes, 10 alcohol swabs, compounded semaglutide drug information, and injection instructions. These additional injection supplies are included in your prescription cost.

If you receive a compounded semaglutide prescription, we recommend that you also purchase a sharps container to throw away used needles and syringes after injection. You can also use a heavy duty empty laundry detergent or fabric softener bottle. Learn more about safe needle disposal.

Potential risks of self-administered injections include:

        • Administering a higher dose than recommended, which can lead to unwanted side effects such as severe nausea, vomiting, and/or hypoglycemia
        • Administering a lower dose than recommended, which can limit the effectiveness or outcome of the treatment
        • Improper use or cleaning, which can lead to infection

If your Hello Alpha provider prescribes compounded semaglutide, it is important to follow your provider's instructions as well as those that will ship with the medication. If you lose the insert or have any questions on how to self-administer, please message your Hello Alpha provider.

        Accordion toggle icon

Before you administer an injection, we recommend that you familiarize yourself with the full instructions for use that come with your prescription. Please also review the instruction video below.

For your safety, do not reuse any needles or syringes. Learn more about safe needle disposal.

How do I safely store compounded semaglutide?
        Accordion toggle icon

Please keep all glass vials in the refrigerator.

Depending on the time of year, your prescription may ship with an ice pack. It is normal for the ice pack to be melted when you receive it. The lab that produces our source of compounded semaglutide has confirmed that its product maintains stability up to 40ºC (104ºF) for up to 5 days. Our fulfillment pharmacy only ships Monday through Thursday to limit travel time and ensure your medication should not be adversely impacted, even if the ice pack has melted. That said, please open the package promptly upon receipt and store all vials in the refrigerator (between 36ºF and 46ºF), keep the vials away from light, and do not freeze or use vials that have been frozen.

Please store all needles and syringes in a secure place where they cannot be accessed by children or other family members.

        • Medullary thyroid cancer. Compounded semaglutide has the potential risk of causing medullary thyroid cancer (MTC). Do not take semaglutide if you have an endocrine system condition called multiple endocrine neoplasia syndrome type 2 (MEN 2). You should not take semaglutide if you have a family history of MTC. If you feel a lump in or on your neck or have painful or difficult swallowing, trouble breathing, or persistent hoarseness in your voice, stop taking compounded semaglutide and seek immediate in-person medical care.

The class of drugs that includes compounded semaglutide may make oral hormonal contraceptives (birth control pills) less effective at certain points during your dosing schedule. If you are a female of reproductive age taking an oral hormonal contraceptive for birth control, we encourage switching to a non-hormonal contraceptive method. Ask your Hello Alpha provider about adding a barrier method of contraception (such as condoms or Phexxi) for 4 weeks after starting compounded semaglutide and for 4 weeks after each increase in dose, which typically occurs every 4 weeks. Alternatively, you can seek care from an in-person OB-GYN to obtain an IUD or implant method of contraception.

Please discuss birth control options with your Hello Alpha provider. Do not take compounded semaglutide if you become pregnant. Compounded semaglutide may harm your unborn baby. Tell your Hello Alpha provider if you become pregnant while using compounded semaglutide. It is not known if compounded semaglutide passes into your breast milk. Please note: During supply shortages, and as an alternative to FDA-approved branded products, where your provider determines GLP-1 medications are appropriate, and where available depending on your state, your  provider may prescribe a compounded drug.  Compounded drugs prescribed by Alpha Providers are prepared by a licensed sterile compounding manufacturer and fulfilled by Alpha's partner pharmacy, in compliance with state law. Although compounded drugs are permitted to be prescribed under federal law, and although the active ingredients compare to brand name medications, compounded medications are not generic, nor are not FDA-approved and do not undergo the same safety, effectiveness, or manufacturing review as branded medications. Zepbound(TM) and Wegovy(TM) are brand name medications that are FDA-approved for weight loss. Ozempic(TM) is a brand name, FDA-approved for type 2 diabetes treatment, but may be prescribed off-label for weight loss at a healthcare provider’s discretion.
